A licensed medical professional who specializes in advocating for the following client health care and lifestyle needs. Here are a few of the services that Country Care Management provides:
• Manages medications
• Establishes clients with home health agencies for skilled nursing & physical therapy services
• Determines medical needs and schedules all healthcare appointments; provides transportation when needed
• Weekly home visits where all needs are assessed by Care Manager. Medication boxes are filled during home visits. Care logs are checked and replaced.
• Manages client health records and patient portals
• Establishes clients with both private and agency daily caregivers
• 24-Hour availability for clients and families to call in the event of emergency or other needs
• Weekly/daily correspondence with families and loved ones via email and phone
• Assists with bill pay and other financial needs as requested
There is no right or wrong time to consider Care Management for yourself or family member. Every client is different, but here are a few indications that it may be time to consider Care Management:
• Inability to manage healthcare appointments or medications
• Family, or yourself, needs support and direction in making educated healthcare decisions
• Family is unable to help daily
• Your loved one or yourself is diagnosed with multiple health issues
• Inability to walk or drive without help
• You feel like you need a strong advocate for your loved one or yourself
• Family and client need supportive and caring help in navigating dementia and other serious medical conditions
• Forgetting to take medications or overdosing on medications
• Unable to shower or meet basic hygiene needs
• Getting lost when driving/forgetting where home is
• Placing everyday items such as keys in unusual locations such as the freezer
• Unable to manage finances or pay bills appropriately
• Increased and debilitating forgetfulness and confusion
• Lack of interest in hobbies and activities or social engagements
• Appliance or stove is left on resulting in fire
• Spouse or other family member is no longer capable of meeting both the daily and medical needs of their loved one
• Alcohol or medication abuse
• New or recent difficulty in standing or walking
• Fall risk when not properly supported by a cane or walker
• Specialized diet or beverage thickener is needed per provider’s order
• Complex medical diagnoses that require long-term care
• Physical therapy or home health is needed
Care Management is mostly a private pay service. Some long-term care policies may offer full or partial coverage. Medicare and insurance do not typically cover any Care Management services.
